The sponsor of this card program is Pethealth Services (USA) Inc

PARTICIPANT
The person(s) named on the card as the purchaser of the membership.

PET
The dog (s) or cat (s) named on the enrollment form submitted by the participant.

MEMBERSHIP

The right to use the card for the purpose of obtaining preferred pricing on veterinary services from network providers.

PROVIDER
A provider or facility holding an agreement with the network to provide quality veterinary services at an agreed upon price to network participants

NETWORK
A preferred provider organization(PPO) engaged in the business of administrating quality veterinary care services at an affordable price through its products.

GENERAL TERMS AND DISCLOSURE

This ShelterCareCard program is not a veterinary health insurance policy.

This plan provides preferred pricing at certain veterinary providers for veterinary health services provided to participants for their pets.

The participant is obligated to pay for all veterinary services directly to the provider. The fees that the provider will charge will be based on the agreed upon price established between the provider and the network. Provider will verify participant’s eligibility and confirm the preferred rates to be paid pursuant to the agreement the provider has with the network.

The program is administered by Pethealth Services (USA) Inc, P.O. Box 2150, Buffalo, New York 14240-2150 The program and its administrators have no liability whatsoever for providing or guaranteeing service or the quality of service rendered by the provider.

In consideration for the purchase of the Sheltercare card membership the sponsor cannot make any guarantee of acceptance of the card at any provider. In conjunction with the network , the sponsor will ensure that all available providers in the network and their address and location will be made available to the participants via a lookup and provider directory which will be updated from time to tie as providers join or leave the network. Providers are subject to change without notice and services may vary in some states. The Sheltercare card program is a membership savings program. Pethealth Services (USA) inc, the sponsor of this program , and the network preferred provider organization are not insurers ,health maintenance organizations or other underwriters of veterinary services. No portion of any provider’s fees will be reimbursed or otherwise paid by the sponsor of this program or the network .The sponsor or the network are not licensed to provide and do not provide veterinary services or items to individuals. You will receive preferred pricing for veterinary services at certain providers who have contracted with the network. You are obligated to pay for these services at the time of your visit to the provider. Savings are based upon on the providers normal fees. Actual savings will vary depending on location and specific veterinary services and products provided. These prices may not be used in conjunction with any other membership savings program. From time to time,certain providers may offer products or services to the general public at prices lower than the preferred prices available to this program. In such event the cardholder is entitled to the lowest price offered. Any procedures or services performed by a non-participating provider who does not have a contract with the network are not at the preferred prices. Preferred pricing on professional services are not available where prohibited by law. Providers may be eliminated from the network by the network provider at its option and the network may substitute new providers at any time. It is the participant’s responsibility to verify that the provider is a participant in the network. The network cannot guarantee the continued participation of the provider in the network. If the provider or facility leaves the network, you will need to select another provider. Providers contracted by the network are solely responsible for the professional services and treatment rendered to the pet(s) and participants and the card sponsor and the network disclaim any liability whatsoever with respect to such matters. Services, providers/facilities may change or be discontinued at anytime without notice.

Renewal Conditions

By purchasing a SheltercareCard membership, you are authorizing Pethealth Services (USA) Inc. to bill your credit card for the plan option you have chosen. This authorization shall remain in force until you notify Pethealth Services (USA) Inc in writing of its cancellation. By purchasing the card, you are agreeing to the SheltercareCard Terms and Conditions and agreeing to purchase the service for a minimum of one year. The participant also agrees that the card will automatically renew at the end of the membership year on an annual basis, and your credit card will be automatically charged or drafted for the appropriate amount.

Termination Conditions

Pethealth Services (USA) Inc. reserves the right to terminate any participant for any reason, including non-payment.

Cancellation Conditions

You have 30 days from the date of plan enrolment to cancel your membership. If for some reason within the 30 days you are dissatisfied with the card program and wish to cancel you may obtain a refund of your membership fees paid less an administration fee of $10.00 provided it has not been used to obtain a discount at a participating provider.
